The website may not have every CGHS form available for download. For some forms you may have to visit the CGHS HQ in your city/town.

There is no specific form for obtaining treatment. Follow the standard course of obtaining treatment from CGHS.
While CGHS will not buy and provide you with a Hearing Aid, whether it will re-imburse you for it (subject to prior approval for purchase) depends on whether an hearing aid is listed in CGHS’s list of approved implants.

I am writing for my father who retired from Border Roads Organization in 2000.
He is diagonised with Multiple Myeloma and was under treatment at Amritha Cancer Institute, Kochi
He is now on expensive Chemotherapy drugs. He is not holding CGHS card at the moment
and is now availing Rs. 100/- per month. We wish to apply for CGHS card now. I have the following doubts.
1. The Bank authorities had advised us to fill up a form to forgo the benefit of Rs. 100/-. I am not able to
find any form. what is the actual procedure to do this?
2.He was not holding an CGHS card while at service.Does he need to provide any proof?
3.He is getting treated in Amritha kochi and the nearest CGHS city is trivandrum. Medicines are also obtained from Amritha.
Medicines are expensive and we would like to get the medicnes through CGHS.
What is the procedure to get the medcines from amritha or any other medical stores?
Can anyone help me please?

w.r.t. (2) & (3), to apply for a CGHS card, the beneficiery needs to apply at the CGHS Headquarters in the city/town nearest to his place of residence. Typically, Photographs, Pension Records and Pension Book (if any) are required. A person may apply for CGHS card post retirement even if he was not a CGHS subscriber during employment.
CGHS only offers treatment in cities/town where they have presence. In you case, your father will have to travel to Trivandrum at-least once/month to get diagnosed at a Govt. Hospital and apply for medicines.
The process of taking my FIL to CGHS Dispensary, Govt. Hospital takes at-least two days. In case, he needs diagnosis and then reports submitted to Govt. Hospital, it adds up-to an additional 5 days. From the day we indent for the medicines, it takes CGHS at-least 3-5 days to issue the medicines. Some medicines are only issued on a weekly basis (i.e. if your weekly medicine bill exceeds 5000/-) and you must re-indent every week (and wait 3-5 days again).
As far as (1) is concerned, I guess you have to write to the Dept. from which you receive the monthly amount for cancellation.

19|Apr|2010 11I am a retired Govt. Employee who retired in October, 2005 in Delhi. During service I was a CGHS beneficiary & had been cotributing Rs.150/- per month towards CGHS. I surrendered my CGHS Card after my retirement in October, 2005 and till date I have not got issued my Pensioner CGHS Card.
Now, I wish to have Pensioner CGHS Card. Will you please be kind enough to guide me the procedure & the amount of Contribution payable for a lifetime ( 10 Years ) CGHS Card. Please guide me.

Please use this document as a sample of the application form.
In short:
- You are required to produce your Pension Payment Order, Age Proof & Residence Proof.
- If you have dependents who should be covered under CGHS, you need to produce Age Proof & Residence Proof for them.
- If you have disabled dependents, Disability Certificate is required
- Photographs of all CGHS beneficieries. Family photograph in case of dependents.
- An application letter is required along with the filled up form
- Please make 3 copies of each document (incl. the application form) before you go to CGHS
- Payment using DD. Calculate your amount by referring to this document.
Sample Contribution: If your salary was greater than Rs. 20,000 per month, then you are entitled to ‘Private Ward’ in a CGHS recognised hospital. Your monthly contribution is Rs. 500/-. Lifetime subscription is Rs. 500 x 120 month = Rs. 60,000.

Please contact the CMO of your CGHS Dispensary to determine if the process you are requesting is available under CGHS. If you are not satisfied with the answer, you may request the ‘Additional Director’ at CGHS HQ in your city for more information/cross verification. If still not satisfied, you can file an RTI application at CGHS HQ and demand to see the list of procedures and medicines covered by CGHS benefits.

CGHS Smart Cards are being implemented in a phased manner across CGHS Wellness Centres in India.
Initially, the impetus is on ensuring that newly enrolled beneficieries of CGHS are issued the ID cards. Plastic ID cards are being issued to existing beneficieries too but as of now they are not mandatory.
You can obtain the CGHS HQ in your city and speak to the Addl Dir. regarding your request to convert your paper-card to a plastic smart card.
