		<description>may i be referred to mumbai by the government doctor of bhopal for treating the acl reconstruction in the following hospital:-

2. Breach Candy, Mumbai
3. Shushruti Hospital,Mumbai
4. cumbala Hills, mumbai.
As per my knowledge, all these hospital are neither empannelled by CGHS nor recognised hospital.
Please clarify this issue and what will the process.</description>

my father is a central govt. employee. CMC vellore used to be the notified referral hospital for the CMPF Dhanbad. he has been under the treatment of CMCH vellore since 2003. and in 2009 he underwent his renal transplant surgery. he used to get an introductory letter from his department addressing CMC that the bills if any be sent to the office for settlement. but in Jan 2011 CMCH Vellore was denotified by the Commissioner and they denied issuing such letter for the treatment. it is mandatory for father to go to cmch vellore in every six months for his follow up treatment. Whether he can claim for medical reimbursement for his recent treatment at cmch vellore? 
We have alreday spent too much money and his survival only depends on the follow up tratment and costly immunosurpassant. going to different hospital for treatment is extremely fatal for him as all his medical investigations and line of treatment are kept there in digital form and the team of doctors there is aware of his medical condition fully well since he has been under their yeartratment for the last 8 years. Please suggest me what could be done in this regard.

sir, i am inspector of income tax at bhopal and i am not covered under 
CGHS while covered in AMA(Authorised Medical Attenndant). i have also two queries:-
First one is that i have anterior cruciate ligament tear. Doctor suggested me to undergo a surgery. i want to undergo surgery with DR. Ananat Joshi, who is a private doctor practising in mumbai. whether it is possible that my medical claim may be reimbursed under AMA. if yes , what will be complete procedure. if not, then what will be helpful if i cover under CGHS.

Second query related to my wife delevery. i have admitted my wife in nearer private hospital in case when she felt labour pain. when i claimed the bill, my account official said to submit the medical emergency certificate. i have read in AMA rule that some relaxation has been provided. one of case is emergency and only it is upto the satisfaction of Head of Department. i wish to know whether the medical emergency certificate is necessary in my case or any other should be advised.

CGHS membership should be taken at a city where you reside permanently. Since you have settled at Bangalore after retirement, I suggest that you approach the CGHS HQ at Bangalore and meet with Administrative Staff (not clinical staff) to determine your eligibility, applicable fees and paper-work that has to be submitted. Things like PPO order/book, Service Certificate, Residence Proof, Identity Proof, Photo/Group Photo of eligible members are sure to be required. Also carry plain white-paper and Carbon paper to make on-spot applications.

Your daughter needs sophisticated treatment which may not be covered by CGHS as a package deal. My suggestion would be to obtain advice from Addl. Director/CGHS in your city. If required, please contact CGHS Helpline and ask for clarification.

Since this is a specialized treatment, you can obtain the treatment at specialist hospitals in Delhi as long as you obtain prior permission.
Another method would be admit your daughter in emergency at a hospital in Delhi, pay the entire costs and ask for partial reimbursement later.

Since rates of private hospitals are many times over CGHS specified rates, the reimbursement on few allowed elements will be as per rates approved for your city only.
